You will be assigned a random number PIN and PASSWORD. (Please remember your PIN and password, the system administrator cannot retrieve lost PINs or passwords.) The PIN and password will enable you to review the survey results again at a later date. The short 42 question survey must be taken in one sitting. If you choose to quit, you may start the survey over at another time using your assigned PIN and Password. The PIN and Password expire in 1 year.

To fill out the response sheet, please read each statement carefully and respond with the number between 1 and 5 that most closely corresponds to your level of agreement with the statement.  If you Strongly Disagree with the statement, use 1; if you Strongly Agree with the statement, respond with a value of 5.  If the statement does not apply to your firm or you do not know the answer, please leave the response blank or choose NA. The average will be calculated on the questions you actually answer (not including NA responses).   

 You will automatically receive the Gap Analysis upon completion of your individual entry. Your score will appear under the Firm Mean.
